Everything You Need to Know About Colombia's Mandatory Health Declaration Form

Before you arrive in Colombia, there's one important (and free) step to take care of: the Colombia Mandatory Health Declaration Form, also known as the Check-Mig form. Here's everything you need to know.

What Is the Check-Mig Form?

Colombia requires all travelers to complete a mandatory health declaration form (Check-Mig) before entering and again before exiting the country. It's a quick online form that must be submitted within a specific time window before your travel. No QR code or printout is needed, but you will need to show confirmation that you've completed the form before boarding your flight to Colombia.

Does This Apply to Me?

This requirement applies to all travelers visiting Colombia. When you'll need to complete it depends on which tour you're booked on:

I'm booked on a tour that starts in Colombia

(e.g., Highlights of Colombia: Bogotá, Medellín & Cartagena)

Colombia is your first destination, so you'll complete the Check-Mig form before you leave home. Since the form can only be submitted within a specific window, plan to fill it out no more than 72 hours before your flight to Colombia and no less than 1 hour before departure.

I'm booked on a tour with a Cartagena extension

(e.g., Highlights of Central America: Costa Rica & Panama with Cartagena extension)

Colombia comes later in your tour, which means you'll be completing the Check-Mig form while already on the road. Since the form can only be filled out within 72 hours of your flight to Colombia, you won't be able to do it before you leave home. Don't worry, your Tour Director will be happy to assist if you need any help completing it on tour!

How Do I Complete the Form?

Head to the official Check-Mig portal: https://apps.migracioncolombia.gov.co/pre-registro/en

Once there:

1. Select "Do Checkmig"
2. Follow the on-screen instructions to complete the form
3. Save your confirmation to show before boarding your flight

When Exactly Do I Complete It?

The form has a strict time window, make sure you submit it:

• No more than 72 hours before your flight to Colombia
• At least 1 hour before your flight to Colombia

Don't forget — you'll also need to complete the Check-Mig form again when leaving Colombia! The same time window applies: no more than 72 hours and at least 1 hour before your departure flight out of Colombia. Your Tour Director can help with this as well.
